Job Summary
SRN is responsible for the delivery of patient care management, treatment and safety of patient under his/her care. He / She has the responsibilities of participating in doctor's rounds by giving relevant information pertaining to nursing observations, laboratory findings and progress of the patient. Ensure charging are carried out accurately and to carry out the duties assigned by the superiors. He / She is also to supervise and guide the subordinates.
Job Responsibilities
As a Team Leader:
- To inform Emergency Department Nurse Manager with regards to any problems encountered. For any problems after office hours or during weekends and public holidays to inform Medical Officer on call, Duty Nursing Administrator on call, and Director of Nursing. (if deem necessary).
- To assist in staff arrangement for the shift in the event that any staff is absent on emergency leave or medical leave.
- To report timely on any malfunction of medical device or facility breakdown.
- To supervise and guide junior nurses and other categories of staff at all times.
- To take up the role of TL during code blue situation in the department till the Code Blue team arrives and as well as for Code Blue event in another department.
- To ensure effective staff mix ratio by planning staff assignment according to their seniority and capability.
- Ensure pharmacy floor stock and consumable items are charged and correctly recorded. In order for adequate replenishment of stocks as per par level set.
- Assess patient's condition in Resuscitation Room is monitored continuously and move stable patient to other zone accordingly to ensure bed availability in Resuscitation Rooms / all 3 zones for emergency case.
- Communicate closely with DNA to ensure bed availability for admission of patient and timely transfer to the respective department to prevent delay in patient treatment.
As a nurse in-charge to direct patient care:
Responsibilities in Resuscitation Room
- To provide quality and safe patient care according to six (6) International Patient Safety Goals (IPSG), CVSKL Quality Improvement and Patient Safety Plan.
- To perform appropriate triaging system for critical and emergency case in Resuscitation room.
- Responsible for the delivery of high quality of nursing care following the assessment of patient
- Participate in Doctor's rounds, chaperon doctor during doctor's examination and to update Doctor regarding patient's condition, nursing observations and diagnostic findings as well as notify doctor promptly when abnormal results in noticed.
- Carry out Doctor's orders accurately and timely.
- Contact Duty Nursing Administrator to source for available bed for patient for admission.
- Sending patient to ward /Critical Care Area /ancillary department or transfer patient to other healthcare facility.
- Ensure and provide assistance to patients on their daily living activity.
- Ensure patient's privacy, dignity and confidentiality is maintained at all times.
- Provide assistance to Radiographer and Cardiac Technician when necessary, during procedure for patient after hours at Ancillary department (if deem necessary)
- Responsible for the preparation of patients for Primary Percutaneous Coronary Intervention (PCI), diagnostic examinations such as CT scan, treatment and surgery. Confirm and ensure that the patient is financially cleared before intervention procedure and surgery.
- Responsible for the proper ordering, collection of clinical laboratory specimens and to be able to detect abnormal results and notify the attending Doctor immediately. To assist Doctor in arranging appointment for referrals to other departments or hospitals.
- Responsible and ensure accurate charging is carried out at every shift in HIS and in order sheet.
- Ensure "Last Office" is carried out properly in accordance to the patient's religious beliefs and customs.
- Carry out all nursing procedures in accordance to work instruction, competency skills and credentialing and privileging approval.
- Provide assistance to Doctors and other healthcare providers during medical examination and procedures.
Responsibilities in Emergency
- Receive, greet and identify patient correctly using 2 identifiers. Place patient in bed/ couch. Provide orientation to patient/ caregiver upon admission, transfer in and understand components of patient's charter as well as conduct appropriate Initial Triage Assessment.
- To identify of any patient's allergies:
- a) Food - To inform food and beverages department and order appropriate diet for patient.b) Drug / Others - To liaise with relevant department e.g., pharmacy
- Ensure allergies status is accurately updated/recorded in EMR as well as the front cover of PMR brown folder.
- Responsible for the delivery of patient care following the assessment of patient. Ensure rapid assessment of pt presenting to ED and prioritize care based on Triaging process according to MTC guideline.
- Reassess patient's condition, plan and provide care of patient accordingly.
- Responsible in ensuring continuity of care by writing complete patient's report and handover detailed report to the staff in the next shift on patient's treatment, care plan, medication and progress of the patient. To complete documentation of Nursing Care Plan before patient's discharge.
- Ensure and provide assistance to patient on their daily living activities. Ensure patient's privacy, dignity and confidentiality is maintained at all times.
- Ensure inpatient prescription, reconciliation form are complete before sending to pharmacy.
- Responsible for the preparation of patient for procedures, diagnostic examinations, treatment and surgery. Confirm and ensure that the patient is financially cleared before intervention procedure and surgery.
- Responsible for the proper ordering, collection of clinical laboratory specimens and to be able to detect abnormal results and notify the attending doctor immediately.
- Perform correct and accurate recording vital signs and intake/output chart of patients.
- Reassure and provide psychological support and health education to patient / care giver during the patient's hospital stay.
- Assist Doctor in arranging appointment for referrals to other departments or hospitals as well as the arrangement for Ambulance service.
- Responsible and ensure accurate charging is carried out at every shift in HIS.To communicate closely with Duty Nursing Administrator on duty for the availability of bed in ward/CCA. Transfer lodging patient to ward/CCA as soon as possible.
- To carry out all nursing procedures in accordance to work instruction and competency skills and credentialing and privileging approval.
- Adherence to infection control guidelines and universal precautions at all time.
- Ensure all nursing documentation in the EMR/nursing clinical template must be completed according to hospital standards.
- To perform triage for patient transfer from clinic to Emergency department for observation and continuous treatment.
As a Medication Nurse
- Responsible for checking upon receiving medication and ensure proper storage of floor stock, psychotropic drug and patient's unit use medication.
- To check the accuracy of the psychotropic drugs together with the hand over shift medication nurse. Responsible to hold the psychotropic drug cupboard keys during the shift and hand over the keys to the next shift staff.
- Responsible for the accuracy and documentation of the administration of psychotropic drugs and the safe keeping of psychotropic drug cupboard keys.
- Responsible to ensure sufficiency of DD by ordering psychotropic drug on the designated days of the ward.
- To follow 7R's during serving /administering medication to patients.
- Ensure medication and injection trolley are kept clean at all times.
- Responsible to monitor the medication fridge temperature daily and to take action if there are any abnormalities in the temperature range.
Responsibilities and Duties relating to indirect patient care
- According to the assignment, to ensure assignment of emergency trolley checks is carried out daily according to the checklist and ensure the resuscitation equipment is ready at all times.
- To check and replenish consumable items after each Code Blue event. Competent in handling life saving equipment such as defib, difficult intubation set, portable ventilator etc.
- Adherence to Personal Data Protection Act Policy (PDPA).
- To assist in conducting department stock take when assigned by those indicated under the authority level.
- Practice good Customer Service Standards with patient, patient's relatives, visitors, superiors and colleagues, subordinates and inter departmental staff.
- Take part in inventory checks and Perform calibration and Quality Control (QC) check for devices/equipment used on POCT.
- Responsible in ensuring Mortuary/body holding cases are handled with respect and in accordance with existing hospital policy and legal aspects.
- Responsible for the transport of deceased body from respective area to Mortuary-Body holding room and to ensure body is handled in accordance to infection control measures and released of body is carried out professionally and accurately to the correct claimant
- Responsible in responding to crisis management promptly and efficiently by adhering to existing hospital policy on managing crisis/disaster.
- Deputize and assumes responsibilities of Nurse Manager in the absence by managing the dept.
General
- To be punctual for duty at all times.
- To be in full uniform attire during working hours. (or appropriate hospital scrubs as per decided by hospital management)
- Active participation and be punctual for the monthly staff /department meeting
- Assist in the proper maintenance of the department inventory, equipment and furniture.
- Participate in quality improvement activities in the department.
- Attend Continuous Nursing Education (CNE) talks organized in the department and Nursing Administration.
- Assist in collecting department data for quality improvement interventions of the department.
- Maintain cleanliness of the unit.
- Be accountable to Any other assignments given by those indicated under the 'Authority level', to be ready to help / assist department with staffing shortages if deployed.
- To fulfil the responsibility as an Emergency Response Team as assigned and take part in the management of crisis by responding promptly and efficiently in accordance with existing hospital policy.
- Adherence to rules and regulations stated in CVSKL Employee Handbook and Code of Professional Conducts for Nurses by Nursing Board.
- With reference to time, the organization shall have the privilege to add-on additional duties and scope.
- To support Customer Service team working towards improving the customer experience and increasing customer loyalty and any other duties as instructed by the management.

- Knowledge in Trauma Management, Cardiothoracic and Cardiovascular nursing
- Proficient in Basic Life Support (BLS)
- Able to write and communicate in English
- IT skill
- Knowledge on Infection Control Guidelines
Professional License or Statutory Requirements (if applicable)
- Annual Practice Certificate by Nursing Board
Abnormal Physical Working Condition
- Ensure the working environment is in good condition with all satisfactory facilities and no hazardous condition that may affect completing of the major duties and responsibilities.
Pay: RM2,200.00 - RM3,100.00 per month
