We are currently looking for Paramedics 5 years post registration for a Primary Care Network in SW London. Ideally with experience of working in the Primary Care setting.
2 days/ week - Tuesday and Wednesday

We are actively seeking an enthusiastic Paramedic who is interested in joining the team in a busy GP Practice in the Croydon area, South London. The role involves:

• Acute and chronic disease management home visiting service
• Care home ward rounds
• One the day clinics for acute conditions
• Advance care planning conversations
• Long term condition reviews
• COPD/ asthma reviews
• ECGs, BP checks and phlbotomy
• Learning disability, dementia health checks
• Visiting house-bound or vulnerable patients
• And much more

You will be an integral part of the Practice team and required to attend clinical meetings.

The Role will require a commitment to register on a First Contact Practitioner pathway (FCP), which you will be fully supported in.

Requirements:

Essential:

• HCPC registered Paramedic since 2021 or earlier.
• Interested in dealing with patient care in the community.
• Willingness to complete essential development and portfolio work.

Desirable:

• Urgent and Primary Care HEE level 6 education
• First Contact Practitioner
• Previous experience in Primary Care
• Previous experience in Primary Care systems (EMIS)
