Job Summary:
Brunswick Street Supportive Housing project provides 24/7, wrap-around support to 38 tenants who are 50+ to age in place. A multidisciplinary team of social workers, nurses, occupational therapist, physical therapist and continuing care assistants ensure each tenant has a support plan that meets the physical health, mental health and social needs. In addition, the team has housing support workers 24/7 to support tenants to maintain their tenancy and access services
The Physiotherapist (PT) is part of the Brunswick Street and NECHC collaborative primary health teams. The PT will work collaboratively with a team of physicians, registered nurses, nurse practitioners, social workers, dieticians, nurses, occupational therapists and non-health professional staff, they will also work alongside an additional 0.5 FTE clinic physiotherapist.
They will offer individual and if warranted, group physiotherapy to individuals who access one or more of the following services at the NECHC: NECHC primary clinic, MOSH, Brunswick Street, and Overlook. In addition to collaboration with staff and volunteers at the NECHC, collaboration with community agencies and systems (health, justice, and income assistance) will be required. Location of work largely at 2445 Brunswick
Street and NECHC primary care clinic (2131 Gottingen Street) with some need for home visits or visits to other community sites.
