Physical Therapist (PT)

Position Overview

Big Minds, Tiny Hands is seeking a compassionate and motivated Physical Therapist (PT) to provide home-based Early Intervention services to children from birth to age three with developmental delays.

This is a fee-for-service position offering flexible scheduling and the opportunity to work directly with children and families in their natural environment. The PT will provide individualized, family-centered therapy services focused on improving mobility, strength, coordination, and overall physical development in alignment with each child’s Individualized Family Service Plan (IFSP).

Responsibilities

Direct Service Delivery

• Provide physical therapy services in the child’s home or community setting
• Implement individualized treatment plans based on IFSP goals
• Address gross motor development, mobility, strength, balance, and coordination
• Use play-based, routine-based, and evidence-based intervention strategies
• Incorporate caregiver coaching to support carryover of skills

Assessment \& Documentation

• Conduct evaluations and re-evaluations as assigned
• Complete session notes daily in compliance with Early Intervention requirements
• Prepare and submit progress reports within required timelines
• Monitor child progress and adjust interventions as needed

Family Collaboration

• Educate families on strategies to support physical development in daily routines
• Maintain consistent communication with caregivers regarding progress and goals
• Provide hands-on guidance to support functional mobility and independence

Coordination \& Compliance

• Collaborate with service coordinators and multidisciplinary team members
• Follow all New York State Early Intervention Program guidelines
• Ensure timely and accurate submission of documentation and required forms
• Maintain compliance with billing and service delivery standards

Qualifications

Required

• Doctorate or Master’s Degree in Physical Therapy
• Active New York State PT License
• Experience working with pediatric population (birth to 3 preferred)
• Strong organizational and communication skills

Preferred

• Experience in Early Intervention or home-based services
• Knowledge of developmental milestones and pediatric motor development
• Bilingual (Spanish or other languages) is a plus
• Familiarity with EI documentation systems (e.g., ProviderSoft)
