Community Health Nurse (RN) – Keele-Rogers Site

Full-time, Contract Position (35 hours per week, Oct. 2026 to Dec. 2026)

$37.53 to $44.46 per hour (salary commensurate on experience)

We are seeking a candidate for the position of Community Health Nurse to join the Primary Health Care team based at our Keele-Rogers site. The ideal candidate for this position will have a strong background in clinical primary care, experience in health teaching, advocacy and health promotion.

Job-Specific Responsibilities:

· Provision of primary care and wellness assessments across the lifespan including immunization, chronic disease monitoring, triage and assessment over the phone, point of care testing, venepuncture, counselling and assessment, and care of minor episodic illnesses in collaboration with physicians, nurse practitioners, and allied health professionals

· Health teaching and advocacy for individuals within a harm reduction framework

· Working collaboratively within a multidisciplinary team

· Participation in team meetings, committees, and centre-wide initiatives

· Performance of administrative tasks.

Job-Specific Qualifications:

· Licensed as a Registered Nurse and membership in good standing with the College of Nurses of Ontario with no restrictions to practice.

· Current RNAO membership with malpractice coverage is an asset.

· Baccalaureate degree or diploma with equivalent experience.

· Demonstrated ability to work effectively independently and as a member of a multi-disciplinary team.

· Ability to work within a community-based context, with a culturally diverse community.

· Experience with Electronic Health Records (experience in Practice Solutions Suite/PSS is an asset).  

· Familiarity with the social, economic and cultural conditions of the project’s geographic area, experience in working with local service providers, community groups and networks, and an understanding of local issues and priorities are an asset.

· Experience serving Unison’s priority populations including people with mental health and substance use issues, seniors with complex needs, LGBTQ2IA+ persons, including persons needing trans care, and youth with sexual health needs facing barriers to access

· Sensitivity to the impact of social health determinants on a diverse, urban, low-income community.

· Sensitivity to, and knowledge of the needs of a multicultural, multiracial, community.

· Demonstrated knowledge of, and sensitivity to, the impact of social, economic, environmental and cultural issues on diverse communities.

· Knowledge of issues affecting communities facing barriers to access in an urban environment.

· Effective communication, interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.

· Knowledge of a second (or more) language and culture is an asset.

· Ability to work one evening a week.
