Job Scope:
- Patient Assessment: Conducting thorough health assessments of elderly clients, including monitoring vital signs and evaluating physical and mental health status.
- Care Planning: Developing individualized care plans based on patients' needs, preferences, and medical conditions, often in collaboration with families and other healthcare professionals.
- Medication Management: Administering medications, monitoring their effects, and educating patients and caregivers about proper medication usage.
- Wound Care: Providing treatment for wounds, pressure ulcers, and other skin issues, ensuring proper hygiene and care protocols are followed.
- Chronic Disease Management: Assisting patients with chronic illnesses by providing education, monitoring symptoms, and coordinating with physicians for ongoing treatment.
- Health Education: Educating patients and their families about health maintenance, disease prevention, and managing specific health conditions.
- Emotional Support: Offering emotional and psychological support to patients and their families, addressing mental health concerns, and promoting social engagement.
- Coordination of Care: Collaborating with other healthcare providers, such as physical therapists, social workers, and dietitians, to ensure comprehensive care.
- Documentation: Maintaining accurate and timely medical records and care notes, ensuring compliance with regulations and standards.
- Emergency Response: Responding to medical emergencies and providing first aid or CPR as needed.
- Advocacy: Advocating for the rights and needs of elderly patients, ensuring they receive appropriate care and services.
- Training and Supervision: Occasionally supervising and training home health aides or nursing assistants in delivering care

Pay: RM2,000.00 - RM2,500.00 per month
