Job Responsibilities

• Provide professional nursing and personal care services in patients' homes.
• Monitor and assess patients' health conditions.
• Administer medications and treatments as prescribed.
• Assist patients with activities of daily living.
• Maintain accurate patient records and nursing documentation.
• Educate patients and their families on proper home care.
• Collaborate with physicians and the healthcare team to ensure quality patient care.

Qualifications

• Bachelor's Degree in Nursing (for nurses).
• BS Midwifery graduate
• Valid DHA, DOH/HAAD, or MOH license or eligibility letter is preferred.
• Home healthcare experience is an advantage.
• Excellent communication and interpersonal skills.
• Compassionate, responsible, and patient-centered.
• Ability to work independently and travel to patient locations.
