Position Summary: Reporting to the Director of Health and Healing, the Licensed Practical Nurse (LPN) Home Care is responsible for providing safe, compassionate, and high-quality nursing care to community members in accordance with the College of Licensed Practical Nurses of Alberta (CLPNA) Standards of Practice. The successful candidate will deliver client-centred care that promotes health, independence, dignity, and an enhanced quality of life.
Working collaboratively within an interdisciplinary healthcare team, including the Registered Nurse (RN), physicians, and other healthcare professionals, the Home Care LPN provides comprehensive nursing services such as client assessments, treatment, medication support, health education, chronic disease management, wound care (within scope of practice), and ongoing home care services. Through a shared-care approach, the Home Care LPN plays an essential role in supporting the health and well-being of community members while fostering respectful, culturally safe, and compassionate care.
Key Responsibilities
Client Care
- Provide safe, ethical, and competent nursing care within the LPN scope of practice.
- Assist with client assessments and contribute to individualized care plans.
- Perform home visits for ongoing assessments, treatments, medication monitoring, and follow-up care.
- Provide wound care, chronic disease management, and health teaching as appropriate.
- Assist clients with Activities of Daily Living (ADLs) according to individualized care plans.
- Monitor changes in client conditions and communicate concerns promptly to the Registered Nurse and other healthcare providers.
- Promote client independence while providing compassionate, culturally safe care.
Clinical Documentation
- Maintain accurate, timely, and confidential client documentation.
- Complete assessments, nursing notes, care plans, incident reports, and required documentation.
- Ensure all client information is maintained in accordance with privacy legislation and organizational policies.
Leadership \& Collaboration
- Provide guidance and direction to Health Care Aides and Homemakers.
- Collaborate with physicians, specialists, community agencies, and multidisciplinary healthcare teams.
- Assist in coordinating specialist clinics and other health services as required.
- Participate in community wellness initiatives, health promotion, prevention programs, and outreach activities.
Safety \& Compliance
- Comply with all Driftpile Cree Nation and Maggie Willier Wellness Centre policies, procedures, occupational health and safety standards, infection prevention protocols, and PPE requirements.
- Follow the Fitness for Duty Policy, including the zero-tolerance policy regarding alcohol and drug use while on duty.
- Maintain client confidentiality and professional nursing standards at all times.
- Report incidents, hazards, and safety concerns promptly.
Additional Responsibilities
- Participate in ongoing education and professional development.
- Support continuous quality improvement initiatives.
- Perform other related duties as assigned to support the effective delivery of Home Care services.
Qualifications
Required Qualifications
- Graduate of an accredited Licensed Practical Nurse (LPN) Program.
- Current registration and good standing with the College of Licensed Practical Nurses of Alberta (CLPNA).
- Minimum two (2) years of related nursing experience (Home Care experience is considered an asset).
- Current First Aid, CPR (HCP), AED, and WHMIS certifications.
- Valid Alberta Class 5 Driver's Licence with a satisfactory Driver's Abstract.
- Strong computer skills including Microsoft Office and electronic medical records.
- Excellent verbal and written communication skills.
- Demonstrated ability to work independently and collaboratively within a multidisciplinary healthcare team.
- Ability to maintain professionalism, sound judgment, and strict confidentiality.
- Knowledge of Indigenous health issues and culturally safe healthcare practices.
- Understanding and appreciation of First Nations culture, traditions, and values.
Assets
- Home Care nursing experience.
- Wound Care Certification.
- Experience with chronic disease management.
- Fluency in Cree.
Working Conditions
- Work is performed in both office and community settings, including client homes.
- Frequent travel throughout the community is required.
- May involve prolonged standing, walking, bending, lifting, and assisting clients with mobility.
- Exposure to various home environments and changing weather conditions.
- May encounter stressful or emotionally sensitive situations requiring sound judgment and professionalism.
- Evening, weekend, on-call, and emergency response work may be required based on operational needs.
