The role \| Mō tēnei tūranga mahi:
As a Community Support Worker (CSW), you will work autonomously with tangata whai ora, supporting them to engage with and remain connected to their chosen community. You will help them work towards self-directed goals, regain independence, maintain wellbeing, and achieve their fullest potential and quality of life. The CSW role includes participating in support and needs assessments, goal setting, and risk management.
The role also involves working collaboratively with whānau/family and within a multi-disciplinary framework alongside Community Mental Health Centres. You will attend regular clinical review meetings with mental health clinicians and other professionals.
