A bit about you
You are a passionate and experienced Registered Nurse who enjoys working autonomously while being part of a collaborative and supportive team. You are confident using your clinical judgement, identifying risks and escalating concerns to ensure our clients receive safe, high-quality care.
You thrive in a role that combines hands-on clinical care with clinical governance, including documentation, risk management and working closely with care managers and other Registered Nurses. You are proactive, organised and technology-savvy, with the ability to identify opportunities to improve care outcomes for older Australians living at home.
You are motivated by making a real difference in people’s lives and supporting clients to remain living independently in their homes with dignity, safety and quality care. You role is not just clinical, it is making time to communicate with the client, perhaps a social chat and cuppa.
Key responsibilities
- Undertake comprehensive clinical assessments and reviews in collaboration with clients and their support networks
- Provide a range of in-home clinical services including wound care, medication management, diabetes monitoring and education
- Develop, review and update clinical documentation and care plans in line with best practice and compliance requirements
- Identify, assess and escalate clinical risks, incidents and changes in client condition
- Manage clinical incidents and ensure appropriate documentation, follow up and reporting
- Conduct regular clinical reviews and audits to ensure quality and safety of care delivery
- Liaise with GPs, pharmacists, allied health professionals and other health providers to coordinate care
- Work collaboratively with care managers and other Registered Nurses to support complex clients
- Delegate and guide care staff appropriately based on clinical needs
- Support falls prevention, risk mitigation and client safety planning
- Provide palliative care support and guidance to clients and families when required
- Provide coverage for another Registered Nurse’s clinical visits during periods of leave
Key selection criteria
- Registered Nurse (Division 1) with AHPRA registration
- Demonstrated clinical experience in aged care, community nursing or home care
- Strong clinical assessment skills and ability to identify and manage risk
- Experience with clinical documentation, incident management and care plan reviews
- Ability to work autonomously, use initiative and prioritise competing demands
- Strong communication and collaboration skills with multidisciplinary teams
- High level of organisation and time management
- Technology-savvy with confidence using clinical systems and documentation platforms
- Compassionate, professional and committed to supporting older Australians
Requirements
- Current AHPRA registration as a Registered Nurse (RN Div 1)
- Experience in aged care, community care, disability or related sector
- Current driver’s licence, reliable vehicle and comprehensive car insurance
- Willingness to travel to client homes across the Gold Coast
- Current Federal Police Check (or willingness to obtain)
