The Specialist Clinical Nutritionist is responsible for delivering evidence-based nutritional assessment, therapeutic dietary counselling, and individualized care planning to patients across both digital and physical clinic settings. The role addresses a wide range of nutrition-related conditions including obesity, diabetes, hypertension, dyslipidemia, and other chronic diseases, supporting patients in achieving improved health outcomes through sustainable dietary change and lifestyle modification. Working as an integral member of the multidisciplinary care team at Bupa CareConnect, the Specialist Clinical Nutritionist promotes preventive health, enhances patient self-management capabilities, and contributes to the delivery of integrated, patient-centered nutritional care.
Nutritional Assessment & Care Planning
Conduct detailed nutritional assessments including dietary history, anthropometric measurements, biochemical data review, and clinical history
Develop individualized, evidence-based nutrition care plans aligned with each patient's medical diagnosis and personal health goals
Monitor patient progress and adjust nutrition care plans based on clinical outcomes and patient feedback
Maintain accurate and complete records of all assessments and care plans in the clinical documentation system
Digital & Physical Consultations
Provide nutritional consultations via telemedicine platforms (video, phone, secure messaging) and in-clinic appointments
Maintain professional communication standards and patient-centered approach across all consultation modes
Ensure seamless continuity of nutritional care between virtual and in-person visits
Document all consultations accurately and promptly in the EMR system with timely follow-up scheduling
Chronic Disease Nutritional Management
Design evidence-based therapeutic nutrition plans for patients with obesity, diabetes, hypertension, dyslipidemia, PCOS, and related conditions
Apply targeted nutritional interventions to support management of each patient's chronic condition
Monitor patient adherence to nutrition prescriptions and track clinical outcomes over time
Coordinate with physicians and care teams for integrated management of complex clinical presentations
Patient Education & Health Promotion
Deliver evidence-based nutritional education on healthy eating, meal planning, weight management, and preventive care
Utilize motivational interviewing and behavior change techniques to support patient engagement
Provide culturally appropriate and patient-centered educational materials and guidance
Follow up with patients to reinforce healthy lifestyle habits and track sustained behavior change
Interdisciplinary Collaboration
Coordinate with physicians, nurses, psychologists, health coaches, and allied health professionals for integrated patient care
Participate in multidisciplinary case reviews and collaborative care planning discussions
Communicate clinical nutrition findings and recommendations clearly to all team members
Support integrated care pathways for patients with complex or chronic nutrition-related conditions
Quality, Compliance & Documentation
Maintain complete, accurate, and timely patient records in the EMR system in compliance with organizational standards
Adhere to organizational policies, clinical governance frameworks, and patient confidentiality requirements
Participate in quality improvement initiatives and clinical audits within the nutrition service
Ensure compliance with SCFHS, CBAHI, and applicable regulatory requirements
Comprehensive nutritional assessment skills including dietary history, anthropometric measurements, and biochemical data interpretation
Therapeutic nutrition planning for chronic disease management (diabetes, obesity, hypertension, dyslipidemia, PCOS)
Telehealth consultation delivery and digital health platform proficiency
Clinical documentation and EMR system proficiency
Evidence-based nutritional counselling and behavior change technique application
Excellent communication and patient education skills adapted to diverse populations

SCFHS classification and active registration as Specialist Clinical Nutritionist / Dietitian
Bachelor’s degree in clinical nutrition, Dietetics, or a related field
