This is a unique opportunity to practise as a palliative care Clinical Nurse Specialist in an innovative, telehealth-enabled model, delivering high-level, specialist support beyond traditional care settings. In this role, you will provide advanced clinical leadership and consultancy to patients, carers, and clinicians managing complex and often escalating needs. You will apply specialist palliative care knowledge to guide clinical decision-making, support virtual care delivery, and ensure patients receive timely, coordinated, and person-centred care across the system. You will play a key role in influencing how care is delivered, supporting integrated pathways, improving access to specialist input, and strengthening the quality and consistency of care across Primary and Community Health and beyond. The role also offers the opportunity to shape the future of service delivery, contributing to capability building, clinical education, and the ongoing development of contemporary models of care within a dynamic and evolving service environment. We are seeking an experienced clinician with advanced palliative care expertise, highly developed clinical judgement, and a strong capability in managing complexity within a dynamic care environment. You will demonstrate the ability to:* Triage and manage complex and evolving clinical presentations with confidence and precision * Deliver high-quality clinical advice and support within a telehealth model * Provide advanced clinical leadership, consultancy, and mentorship to clinicians across the system * Navigate and coordinate care across multiple services, ensuring seamless and integrated patient pathways * Adapt to and positively influence an evolving model of care

To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.* Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA). Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation. * Describe a time where you were required to assess and prioritise a patient’s needs without face-to-face contact. How did you identify and escalate clinical risk, apply your specialist knowledge, and ensure safe, timely, and appropriate care? * Provide an example of how you have supported a patient or carer experiencing distress, uncertainty, or complex palliative care needs. How did you tailor your communication, and what strategies did you use to achieve a compassionate, person-centred experience? * Describe your experience navigating patients across health and/or community care systems. How have you applied your clinical judgement and influenced decision-making to coordinate care, connect patients to appropriate services, and reduce fragmentation or delays? * Tell us about a time you worked collaboratively within a multidisciplinary team during a period of change or service development. How did you demonstrate clinical leadership, support and influence other clinicians, adapt your practice, and contribute to improving the model of care or service delivery?
