Advance Care Planning Facilitator

Canterbury Waitaha is looking for an Advance Care Planning (ACP) Facilitator to support the development and integration of Advance Care Planning across primary care, community, aged residential care and hospital settings.

About the role

As an Advance Care Planning Facilitator, you will help strengthen ACP practice across Canterbury Waitaha by supporting clinicians, providing education and mentoring, reviewing documentation and promoting high-quality Advance Care Planning and Shared Goals of Care.

You will work collaboratively with a wide range of health professionals and organisations, helping to build confidence and capability in having meaningful conversations about what matters most to people and how they would like to be cared for.

Key aspects of the role include:

• Supporting the implementation and integration of Advance Care Planning across primary care, hospital and aged residential care settings.
• Providing guidance, coaching and support to clinicians undertaking ACP conversations.
• Reviewing ACP documentation to support quality, consistency and clinically actionable plans.
• Delivering ACP and Serious Illness Conversation Guide education, workshops and professional development.
• Supporting the development of ACP champions within clinical teams.
• Promoting awareness and uptake of ACP across the health sector.
• Collecting and reporting activity and outcome data and contributing to audits, evaluation and quality improvement.
• Developing strong relationships with clinicians, aged residential care providers, community services, palliative care and other ACP stakeholders.
• Contributing to the ongoing development of ACP resources, processes and best-practice approaches.

This is a 0.5 FTE position (20 hours per week) for a Fixed term starting ASAP until the 30 June 2027 reporting to the Clinical Lead, Advance Care Planning, within the Canterbury Initiative.

About you

We are looking for a registered healthcare professional who combines sound clinical judgement with excellent communication and relationship-building skills.

You will have:

• A relevant graduate and postgraduate qualification.
• Current registration and a practising certificate as a healthcare professional.
• Experience working with older people and their families/whānau.
• An understanding of, and genuine passion for, Advance Care Planning and its intent.
• Excellent verbal and written communication skills and a high level of verbal reasoning.
• Strong critical thinking and clinical reasoning skills.
• Excellent interpersonal and relationship management skills.
• Experience working across different clinical settings and with a range of stakeholders.
• An understanding of the significance of Te Tiriti o Waitangi and how to apply its principles meaningfully in your mahi.
• A commitment to person-centred care and equitable health outcomes.

It would be an advantage if you have:

• Completed a Level 1A Advance Care Planning workshop.
• Experience as a Practice Nurse and/or working in an aged residential care facility.
• Knowledge of Shared Goals of Care processes.
• Experience delivering education, facilitating workshops or supporting professional development.
• Experience with systems such as Health Connect South, HealthOne, Microsoft Teams and clinical databases.
• Knowledge of quality improvement methodologies.
• Strong IT skills and confidence working with a range of digital systems.
