Patient Care and Monitoring: Assist with daily activities such.
Communication and Documentation: Educate patients and families on health conditions and treatments.
Emergency & Critical Care: Respond quickly to emergencies

Responsibilities & Context

Call patients and monitor patients health conditions over the phone.
Keep accurate patient records and document treatments.
Communicate with patients and families about care plans over the phone call.
Over the phone call provide guidance on medication adherence and side effects.
Educate patients and families on health conditions and treatment.
Advocate for patient needs and well-being
